Course Description
This course is a part 2 of a 2 part requirement for the Year 12 Local Health Department (LHD) Performance Incentive Program (PIP)
Course Description:
Part 2 of the Year 12 LHD PIP is intended to help LHDs to develop an action plan based on the Center for Disease Control and Prevention's Active Communities Tool (ACT). The training will discuss the importance of an action plan to help document action steps for creating activity friendly routes to everyday destinations. The webinar will help LHD staff learn about action planning, understand the process for and resources needed to complete an action plan, and to understand the expectations for completing an action plan and submitting with supporting evidence to the New York State Department of Health.
